For patients with neurological and complex trauma conditions, Hope Network Neuro Rehabilitation has developed Rehabilitation Solutions – strategies that address common problems often encountered in care management. Each strategy targets specific goals in a specialized, structured and high-intensity rehab environment.
These solutions are categorized into four areas:

Supervision Reduction
Interdisciplinary collaboration, with a focus on reducing the amount and type of care required by the patient with a brain injury, allowing discharge to a targeted placement or placement level.

Psychiatric Stabilization
Short-term residential neurobehavioral-focused therapy that aims to stabilize a patient’s psychiatric functioning allowing them to return to their prior level of care.
03.
Complex Medical
Interdisciplinary, focused, and intense therapy solutions that concentrate on improving and restoring the functional capacity of an individual who has complex medical issues as well as a neurological condition.
04.
Care Transitions
Solutions focused on health stability and risk management when transitioning individuals with high care needs to their next level of care. The primary goal is to educate and engage caregivers on general caregiving and injury-specific topics to ensure a smooth discharge transition no matter the placement.
